Please answer "Yes" or "No" to the following eight questions:- Have you been hospitalized in the last five years?
- Have you been treated for cancer, a stroke, a heart attack, or TIA?
- Have you had any problems with your heart, lungs or circulation?
- Have you been diagnosed with diabetes, Parkinson's, or emphysema?
- Have you ever been diagnosed with Alzheimer's, dementia, or memory loss?
- Do you need any assistance walking, such as the use of a wheelchair, walker, or cane?
- Do you have any surgery planned?
- Are you currently taking any prescription medications?
