How to Begin a Claim by Phone

When calling, you will be asked to provide the following information:

  • Decedent’s full name
  • Date of birth
  • Social Security Number
  • Place of death (U.S or Foreign)
  • Policy or Contract number (if known)
  • Date and Cause of death
  • Name of person reporting the claim, and his/her relationship with deceased
  • Daytime and evening telephone numbers
  • Mailing Instructions for claim packets
